Hi, I'm sorry for the last minute entry. Things have been hectic. I had to go perform at a candidate debate today. I've attached a quick draft of the survey.

Unfortunately not enough time has been given to add my input thoughtfully.
My first reaction is that its too confusing.
Gym part must be a stand alone survey. Proposal # 2 is all hypothetical. When a million dollar or thousands of $$ are mentioned it would naturally spook the reader or a survey taker to go against it
Proposed 55+community to be a stand alone survey.
I want to add more to this but no time left.
